Stratford Academy Parents,
Please read the information below as a reminder of the forms that are required for enrollment for all new students and the updated immunization records that are required for all new students and for ALL Beginners, PreKindergarteners, Kindergartens, 5th Graders, 7th Graders, and 11th Graders. It is especially important that we receive up-to-date immunization records for students in the grades listed above ON or PRIOR TO the FIRST DAY OF SCHOOL. Students who do not have up-to-date immunizations will not be permitted to attend school until they do. If you are unsure of your child's immunization record, please contact your child's pediatrician. Please do not wait until the last minute to do this! We have to have these forms no later than August 18th (7th & 11th Graders), August 19th (5th Graders), or August 23rd (Beginners, PreKindergarteners, and Kindergarteners).

You will also have to complete an updated Medical form, OTC medication permission slips, and ASAP forms (PS & LS families); these forms will be sent home on the first day of school and will be available through the My Stratford portal. They are separate from the documentation described here.

REQUIRED DOCUMENTATION * ALL students must have on file in the Registrar's Office a Georgia Department of Public Health Certificate of Immunization (Form 3231). This can be obtained from your child's pediatrician or at the Macon/Bibb County Health Department. You may bring this form with you on the first day of school, have it faxed to us at 478-477-0299, email it as a pdf to marilyn.walker@stratford.org, or mail it to Stratford Academy to the attention of Marilyn Walker, Registrar. Without this form on file by the first day of school, students will not be allowed to remain at school. (This form must be updated each year in Beginners, PreK, Kindergarten, 5th Grade, 7th Grade, and 11th Grade. You can call your pediatrician's office to determine if your child is current with his/her immunizations.) Students are required to be immunized to attend Stratford Academy. * ALL students must also have on file in the Registrar's Office a copy of their Birth Certificate and Social Security Card. Please bring, fax, email, or mail a copy of these forms to the attention of Marilyn Walker, Registrar, as soon as possible. We would like to have these forms for all students no later than Labor Day. * In addition, all students who are PreKindergarten or older must also have on file in the Registrar's Office a Georgia Department of Public Health Certificate of Vision, Hearing, Dental, and Nutrition Screening (Form 3300) when they begin PreK or when they initially enroll at Stratford (for students older than PreK). (Once Form 3300 is submitted to the school it does not have to be updated). Please help us by getting these required documents in to the Registrar's Office as soon as possible! We would like to have these forms on file for all students for whom they are required by Labor Day. Thank you!
