IMMUNIZATION AND HEALTH FORMS
Georgia law requires that all students entering Georgia schools for the first time have a certificate of immunization form on file at the school no later than the first day of school.
All Beginners (3 year olds) and Pre-Kindergarten students must have immunization form 3231 on file in the Preschool Office. Also, please furnish the school with a copy of his/her birth certificate and social security number.
Kindergarten students must have form 3231, as well as, form 3300 (Certificate of Ear, Eye, and Dental Examinations) on file in the Preschool Office. We are also requesting a copy of the birth certificate and social security number for the school’s records.
All new students (grades 1 – 12) must also have a current immunization record, a copy of their birth certificate, and their social security number on file in the registrar’s office no later than the first day of school.

Please send an updated copy of your child’s immunization form to the registrar, when additional vaccines are administered.
